New Patient Registration
We are delighted to welcome new NHS patients to our Practice. We do not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
Do you live within our boundary area?
Every GP practice has a specific registration area and our area map is displayed below. We will happily register any patient who lives in this area, please note you may be asked for proof of address. If you move, but remain in our practice area we can continue to be your GP, please let reception know of your new contact details.
How to Register
We are using a new online service that makes it easy to register with this GP surgery.
Just fill in this quick online form.
When you register, you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please fill in the whole form as accurately as possible.
You do not need proof of address or immigration status, ID or an NHS number to start registering with a GP surgery.
It's easy and will save everyone time.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Temporary Services
If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.
To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
Your Named GP
The surgery provides all patients with a named usual GP who will have overall responsibility for the care and support that our surgery provides to them. We encourage patients to try and see their usual GP where possible, particularly for ongoing problems.
We need to divide our patients evenly between all of our doctors, so they can manage their workload. Therefore the registering GP rotates from week to week based on each GP's list size. We try to keep families under the same usual GP where possible, so new babies will be registered with their mother's usual GP.
If you wish to be told the name of your usual GP, please ask the reception team when you are next in the surgery.
